How to Book a Transfer

MedicVan – Patient Transportation is available 24 hours a day, 7 days a week.
If you would like to book a transfer, simply call our office and speak with our dispatcher, who will be happy to assist you. You may pre-book your transfer days, weeks or even months in advance.


Please have the following information available when you require a transfer:
  • Caller’s full name and phone number
  • Patient’s full name, age, weight, diagnosis and the rate of oxygen required if applicable
  • Patient’s Precaution (MRSA+ ETC.)
  • Patient DNR order, if applicable
  • Pick-up location, floor and room number, if it’s a house the number of stairs
  • Preferred date and time of transfer
  • Destination address, floor and room number
  • Appointment time and length of appointment
  • Request for return or assistance trough the appointment
  • Any request for escort
  • Payment arrangements
  • MT or PTAC# if applicable

Method of payment:

  • We accept Cash, Visa, Master cards.
  • WSIB, Insurance Plans welcome.
